Eight-hour long surgery; detached hand reattached back

~ Amputated hand of a Tamil Nadu native was reattached back successfully after 16 hours, through microvascular surgery at KIMSHEALTH, Thiruvananthapuram ~

 

Thiruvananthapuram, April 17, 2023: A 30-year-old man whose hand got detached from his body in an assault, came to KIMSHEALTH Thiruvananthapuram where the hand was reimplanted back into the body. In addition to the hand injury, the large blood clot in the brain put the young man’s life in danger. Surgeries that lasted more than eight hours incorporating multiple specialities, which started with neurosurgery, were followed by the hand and microvascular surgery to reattach the hand. It was through the timely intervention of the medical team that the young man was able to return to normal life even 16 hours after the accident.

 

The young man who sustained an assault wrongly in a local conflict was brought to the hospital, 8 hours after the injury. Normally detached limbs that have been cut off from the blood supply should be reattached within 6 hours, which is called the golden period. Here, in addition to the fact that he had lost valuable time before coming to the hospital, still had to wait before the hand & microvascular team could reattach the hand since he had life-threatening injuries in the brain. 

 

"The patient, who was in a state of fractured skull pierced into the brain, underwent a neurosurgery that lasted an hour and a half through which the broken skull bone was removed and was fixed in normal position using a titanium mini plate,” said Dr. Ajith R, Senior Consultant, Department of Neurosurgery. Simultaneously, preparations to reimplant his severed hand were also progressing in another operation theatre.

 

Under the leadership of Dr. Manoj Haridas, Hand and Microvascular Surgeon, and Senior Consultant, KIMSHEALTH, the surgical procedures to reattach the detached hand began. Dr. Sherly John and Dr. Valli from the Anesthesia Department were also part of the team that performed a non-stop operation to restore the man’s hand by fixing the bones and repairing back the severed arteries, veins, tendons, and nerves. This was followed by the orthopaedic surgical team's fracture fixations in the patient’s leg.

 

By the time the microvascular surgery began, about 12 hours had already elapsed as necessary blood tests had to be done and the patient had to be made fit for major anaesthesia following 6-hour fasting protocols. Although 16 hours had passed when the blood flow was restored to the amputated hand, with the thumb of the hand in  a stiff rigor mortis state. Yet, getting the hand to survive without any complications is a rare achievement in the field of hand and microvascular surgery, said Dr. Manoj Haridas. The patient will get back to optimum function after three to four months of dedicated hand physiotherapy under expert guidance. Dr. Ajith R, Senior Consultant, Department of Neurosurgery, Consultant Neurosurgeons, Dr. Abu Madan, Dr. Navas N.S, Dr. Bobby Iype, and Neuro Anesthetist, Dr. Susanth B. were part of the neurosurgical procedure.
